This email is solely intended for the purpose of requesting general information about OATC. Please do not include personal medical information as it is a general mailbox for the purposes of information. We will not respond to individual treatment requests, missed appointments, appointment changes etc. You must visit your clinic for such needs.

Concerns -

There are a variety of ways you can express your concerns about your experience with OATC. All concerns must be made in writing and delivered either in person by making an appointment with CCM or physician, email, mail, fax or phone.
